Achot, that is correct.
I noted that I would like to be able to go to the source/rav, to learn his rationale for the particular circumstances. This is little different from when I learn responsa, except in this case I have the advantage of being able to further question the source, and/or challenge his rationale.

Update here. I'm a practicing doula now (doing a little dance) and I was told by my rabbi that I may take a cab to a birth on shabbos and may return in the same manner EVEN for a non jewish client.

By the way, i am planning for my 3rd birth and definitely want a doula with me. Not sure if it's going to be some Jewish as I am due on pesach. Still just wanted to emphasize that i still feel it necessary even for multipara.
